Cable & Internet Set-Up and Service Issue Form

Use this form to request the initial set-up for cable and internet service . If you are a new owner, you must have completed the New Owner Request Form before your cable and internet can be turned on for your unit. This should have been completed before closing. If the POA does not have a copy of your new owner form, you must fill one out.

OR

Use this form to set-up your Optimum Account OR report a service issue. The POA has a bulk service agreement with Optimum, which represents significant savings for our owners. However, once service has been turned on in your unit, you are required to work with Optimum directly, which requires an account number and pin number.

If you already have an Optimum Account set-up and are having issues with your service, you will need to contact Optimum directly using your account number and pin number.
