Cable & Internet Set-Up and Service Issue Form
Use this form to request the initial set-up for cable and internet service . If you are a new owner, you must have completed the New Owner Request Form before your cable and internet can be turned on for your unit. This should have been completed before closing. If the POA does not have a copy of your new owner form, you must fill one out.
OR
Use this form to report a service issue, and a board member will work with our Optimum rep to resolve the issue.
*NOTE – If a technician needs to visit your unit to resolve the issue, an adult over the age of 18 WILL need to be present for the service call. The owner or property manager is responsible for ensuring someone is available. Resort staff will not be used for this purpose.
